Our Wellbeing Team supports patients with their emotional health and overall wellbeing, helping people feel more supported, connected, and able to manage everyday challenges.
This is a non-clinical service, open to all patients, including:
Unpaid carers
People living with or affected by cancer
Those experiencing stress, low mood, isolation, or life pressures
The team can:
Offer time to listen and understand what matters to you
Help you explore practical and emotional support options
Signpost to local groups, services, and community support
Support you to manage long-term conditions alongside day-to-day life
Our Wellbeing Team is made up of Mel, Mo and Jodie. Who also work closely with Ashia, our Carer's Lead.
Although the team is part of the Primary Care Network (PCN), they are fully integrated within our practice and work closely with our GPs, nurses, and wider team.
The team work to offer a range of wellbeing initiatives and support options, designed to meet the needs of our local community.
The Wellbeing Team focuses on what matters to you, not just your health conditions.
