If you have high blood pressure (hypertension), it’s important to have a review at least once a year. Regular reviews help us monitor your blood pressure, reduce the risk of complications, and support you in managing your condition effectively.

 

What is the hypertension review for?

The review allows us to:

  • Check how well your blood pressure is controlled

  • Reduce the risk of heart attack, stroke, and kidney disease

  • Review your medications and treatment plan

  • Offer advice on lifestyle changes that can help lower blood pressure

  • Answer any questions or concerns you may have

 

Annual reviews are recommended by the NHS and are an important part of long-term blood pressure management.

 

What happens during the appointment?

Your hypertension review is usually carried out by a practice nurse or healthcare professional and may include:

  • Blood pressure measurements

  • A blood test

  • A urine check
  • Review of your medications and any side effects

  • Weight and lifestyle discussion (diet, exercise, alcohol, smoking)

  • Assessment of cardiovascular risk

  • Planning follow-up or additional tests if needed

 

What to bring with you

Please bring:

  • A list of your current medications

  • A urine sample
  • Your home blood pressure monitor or readings, if you have them

  • Any questions or concerns you would like to discuss

 

Regular blood pressure reviews help us identify problems early and support you in staying healthy.
