Registration

Patients are registered with the Practice, not an individual GP. Your medical card will be issued in the name of one of the doctors who will supervise your care. However you can at any time express a preference for a practitioner, for either all of your medical needs or on a case by case basis. This preference will be recorded in your medical notes. Whilst the Practice will always try to comply with your requests on all occasions, it may not always be possible.

Temporary Residents

You may consult a doctor anywhere in the UK if you are away from home and in need of medical attention. You can do this by asking to see the nearest doctor as a temporary resident. Likewise, either doctor will be pleased to see anyone staying with you upon the same basis  

Non-English Speakers

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
* Please note that the Practice reserves the right under various circumstances to refuse an application.  The registration is not valid until  it has been agreed and the applicant has visited the Practice to sign the form.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
